ABSTRACT

Objective:To investigate and analyze the allocation and use of the common pediatric medical equipment and drugs in community health service centers, so as to provide decision-making reference for further strengthening the capacity-building of community children′s health services.Methods:Using multi-stage stratified random sampling method, 30 community health service centers in 14 cities were selected to investigate the basic situation of the sample centers, and the allocation and use of 21 kinds of commonly used pediatric medical equipment and 23 kinds of commonly used children′s drugs from April to August 2020.Results:The overall allocation rate of 21 commonly used pediatric medical equipment in the sample centers was 61.9%, and the overall utilization rate was 62.6%. The overall allocation rate of 23 commonly used drugs for children in the sample centers was 46.67%, and the overall utilization rate was 31.1%. There was a statistically significant difference in the utilization rate of equipment among different levels of urban community health service centers ( P<0.05); There were statistically significant differences in the allocation rate of commonly used children′s drugs among community health service centers under different regions, cities at different levels, types of jurisdictions, and institutional types ( P<0.05). Conclusions:The overall allocation and utilization rate of commonly used pediatirc medical equipment and drugs in urban community health service centers in China were relatively low, especially the drug utilization rate. The author suggested that the government should further strengthen the construction of grassroots pediatric diagnosis and treatment service capabilities, and reasonably equip commonly used pediatric medical equipment and drugs at the grassroots level.

ABSTRACT

Objective:To provide strategic suggestions for optimizing children′s diagnosis and treatment services in the communities, by means of analyzing the overall efficiency of children′s diagnosis and treatment services in the sample community health service centers, and learning the current input and output of children′s diagnosis and treatment resources.Methods:In April 2020, a total of 27 community health service centers in 14 cities were selected by random sampling. Data such as the number of medical visits by children aged 0 to 18 years and the area of pediatric diagnosis and treatment departments in the sample centers in 2019 were collected by self-filling questionnaires. Excel was used for data sorting. Data envelopment analysis(DEA) was used for data processing. The data processing tool was DEAP 2.1.Results:The average comprehensive efficiency, the average technical efficiency and the average scale efficiency of the 27 sample community health service centers were 0.445, 0.865 and 0.494 respectively. There were five DEA efficient centers, 4 DEA weak inefficiency centers and 18 inefficient centers. Six out of 18 DEA inefficient centers had redundant input of healthcare professionals capable pediatrics; 12 centers were short of children visits, and 15 were short of visits by children aged 0-6 years.The centers where DEA was inefficient were concentrated in the central region, the suburbs and " centers with independent pediatric clinics but without pediatric wards" .Conclusions:The comprehensive efficiency of children′s diagnosis and treatment services in the sample community health service centers is relatively low. Currently, the sample community health service centers are faced with such problems as small and insufficient input of children′s diagnosis and treatment resources in the community, unbalanced development of children′s diagnosis and treatment services in the region among others. It is suggested that on the basis of making full use of the existing resources to create the maximum output value, we should consider appropriately expanding the scale of resource input to improve the efficiency of children′s diagnosis and treatment services at the primary level and further give play to the value of the " gatekeepers" at the primary level in children′s diagnosis and treatment.

ABSTRACT

To prevent and control the epidemic of coronavirus disease 2019 (COVID-19), the community health service center should take the initiative to cooperate with sub-district office and police station to form a joint working team. Taking the advantages of professional expertise and using service contract system, the general practitioners can publicize the epidemic prevention knowledge and self-protection methods among community residents; and also take door-to-door inspection to make the decision of home isolation, centralized isolation, fever clinic visit or transfer to the designated hospital for observation and treatment. During the COVID-19 epidemic, we should give full play to the enthusiasm initiative of the community, general practitioners and residents to implement prevention and control measures and to ensure the health and safety of the people.

ABSTRACT

Since December 2019, the outbreak of coronavirus disease 2019 (COVID-19) has rapidly swept across the country, posing great challenges to the public health in China. In the epidemic prevention and control, primary care practitioners play a very important role in patient triage, home-based visit, follow-up as well as screening at the checkpoints. However, due to the lack of necessary protective equipment and heavy workload, primary care practitioners are facing great challenges in containing the epidemic outbreak. Based on the relevant guidelines and practice in primary care, this article summarizes the challenges primary care practitioners encountered and coping strategies for containing the epidemic outbreak in primary care settings to provide reference for improvement of their working quality.

ABSTRACT

The community is the first line of endemic prevention and control. Facing the challenge of epidemic outbreak, Fangzhuang Community Health Service Center applied the intelligent family doctor model to carry out joint community prevention and control of coronavirus disease 2019 (COVID-19) epidemic. The intelligent family doctor model is a people-centered, information-based approach, which integrates community resources to carry out health education, counseling and public opinion guidance. With this model the community health service center can effectively screen and follow up the key groups to strengthen the joint prevention and control of COVID-19; at the same time it also can manage the chronic disease and the elderly care. This article introduces the intelligent family doctor model, and analyzes its advantages and existing problems, to provide references for community epidemic prevention and control.

ABSTRACT

Since the key point of prevention and control of new coronavirus infection (COVID-19) in the community is to block the spread, the centralized quarantine for close contacts with confirmed or suspected cases is the most effective way in breaking the chain of transmission. A community centralized quarantine site was set up by Fenglin Community Health Service Center for medical observation of close contacts. The detailed plan and management of the centralized quarantine site, and the implementation experiences are introduced in this article to provide reference for medical institutions involved in the prevention and control of COVID-19 epidemic.

ABSTRACT

Objective To observe the advantages and disadvantages the combination of traditional Chinese medicine and western medicine in community health service centers through cost-effectiveness and cost-utility analysis.Methods Take community common diseases as an example, Knee Osteoarthritis (KOA), Cervical Spondylosis (CS) and Lumbar Disc Herniation (LDH) were used. A total of 200 cases were selected for each disease, and 100 cases in each one group.In the treatment group, the rehabilitation mode was used.In the control group, comprehensive treatment of traditional Chinese medicine was adopted. We analyzed the cost-effectiveness and cost-utility, with VAS as the effect indicator and QALYs as the utility indicator. Results The VAS values of the knee osteoarthritis, cervical spondylosis and lumbar disc herniation in the treatment group were lower than those in the control group after 14 days, 30 days and 6 months (P<0.05) . The QALYs values of them in the treatment group were lower than those in the control group after 14 days, 30 days and 6 months (P<0.05) . The treatment group has a higher increase cost-effectiveness ratio (ICER) . Each get a unit effect can pay 222.56, 297.13 and 1178.59 Yuan less . The treatment group has a higher increase cost-utility ratio (ICUR) . Each get a unit effect can pay 741.86, 3178.60 and 2862.29 Yuan less . Conclusion Community application in rehabilitation mode of integrated traditional Chinese and western medicine can improve the treatment effect, and can reduce the cost of treatment and the loss of QALYs.

ABSTRACT

Objective To learn the current doctor-patient trust in contracted service of family physicians in suburban Beijing, and analyze its influencing factors from residents′ perspectives to propose countermeasures. Methods A total of 197 contracted residents were sampled from 12 community health service centers in 4 out of 10 suburb districts of Beijing, for a questionnaire survey using stratified random sampling method. Items of the questionnaire cover residents′knowledge of the family physicians′ contracted service, their trust of family physicians′ competence, and their trust level of the ethics and the influencing factors. Measurement data were calculated form the mean value, while the count data were subject to proportion analysis, frequency description and rank ordering. Results 175(88.8% )and 156(79.2% )of the contracted residents trust the ethics and competence of their family physicians. Whether the ethics trust exists between doctors and patients was decided by if residents′respect, care, and interests considerations taken into account by the physicians; the factors influencing the competence trust of the residents were the age, education of the residents, and whether the physicians display respect, care. Conclusions Upgrading the medical competence and ethical conduct is conducive to the establishment of doctor-patient trust. In the establishment of the family physicians contracted service, it is necessary to improve acceptance of the contracted residents. At the same time, medical ethics issues such as respect for care, privacy protection, and maximum interests of the residents in the process of medical treatment deserve further attention.

ABSTRACT

OBJECTIVE:To provide reference for the construction of suitable evaluation index system for the implementation of National Essential Medicine System in community health service center.METHODS:The frame of evaluation index system was designed by theoretical analysis,literature study and expert interviews methods.Delphi method was used to establish evaluation index system for National Essential Medicine System in Shanghai community health service center.Analytic hierarchy process was adopted to determine the weights of indicators at all levels.RESULTS:The positive coefficients of the two rounds consultations were 88.24% (15/17) and 86.67% (13/15).The authority coefficient of structural index,processing index and result index in the first round consultation were 0.68,0.68 and 0.69,respectively,which showed that the authority degree was higher.The coefficient of concordance in the second round consultation was higher than the first round (the indexes of structure,process and result in two rounds consultation were 0.286,0.306,0.193 and 0.345,0.358,0.337,respectively),which indicated the expert opinions were highly consistent.Established evaluation index system consisted of 3 fiust-level indexes,13 second-level indexes and 27 third-level indexes.CONCLUSIONS:The selected experts were representative,the consultation results were reliable,established evaluation index system was rational and the setting of weights was scientific.Also this study was a methodological exploration.

OBJECTIVE:To provide reference for the further promotion ofNew Medical Reformand the further implementa-tion and improvement of National Essential Medicine System. METHODS:By systematic sampling,1728 outpatient prescriptions were randomly selected from a community health service center in Foshan during 2012 to 2015,and then analyzed and compared statistically in respects of the amount of drugs,rational drug use indexes,prescription cost,main disease composition,treatment cost. The change rules and characteristics of all the above mentioned information were all studied. RESULTS:The average number of medicines per prescription and the number of essential medicines decreased year by year,Comparied with 2012,the proportion of antimicrobial agents,the proportion of two or more antimicrobial agents,the proportion of injection prescription and the propor-tion of transfusion prescription were significantly reduced,the cost of medical treatment of patients decreased significantly;medi-cine cost and medicare reimbursement amount had increased to different extents,there was statistical significance(P<0.05). Multi-ple linear regression analysis(stepwise)showed that the cost of medical treatment was closely related to rationality of prescriptions. Respiratory diseases,cardiovascular and cerebrovascular diseases,gastrointestinal diseases were the top 3 of the major diseases, the proportion of prescriptions for respiratory diseases,gastrointestinal diseases decreased year by year,while that for cardiovascu-lar and cerebrovascular diseases increased year by year. CONCLUSIONS:The promotion ofNew Medical Reformand the imple-mentation of National Essential Medicine System play a positive role in promoting rational drug use and the quality of medical ser-vice in community health service center. However,there are still many problems in the community health service centers,such as too many medicines each prescription,the lack of diagnosis and treatment ability,the defects of medicine selection and administra-tion.

Objective@#To analyze satisfaction with follow-up management of HIV/AIDS cases conducted by Community Health Service Center (CHS) and related factors in Hongkou district, Shanghai.@*Methods@#Out of 302 HIV/AIDS cases followed up by CHS in Hongkou district from 2012 to 2016, 263 HIV/AIDS cases were recruited to participate in a cross-sectional study assessed by self-designed questionnaire-based interview during October 1, 2016 and October 20, 2016, with efficiency of 87.1%. Information of basic characteristics including sociodemographic, routes of infection, CD4+T cell counts, diagnose of AIDS and status of receiving ART were collected, as well as satisfaction with follow-up management conducted by CHS including service professionalism, service attitude, service environment, referral service, privacy protection, health education, psychological support, and care and assistance. Factors related to satisfaction were analyzed using multiple binary logistic regression.@*Results@#Among 263 HIV/AIDS cases, the average age was 42.0±13.5, 93.2% (245 cases) were male and the proportion of overall satisfaction was 72.2% (190 cases). Out of 8 items of satisfaction, service attitude and health education got the highest score with a total number of 235 (89.4%) answering "very satisfied" or "satisfied" , while care and assistance got the lowest score with a total number of 69 (26.2%) answering "dissatisfied" or "very dissatisfied" . Compared to HIV/AIDS local residents and followed up by CHS <12 months, those who were non-local residents and followed up by CHS ≥12 months were more likely to be satisfied, the OR (95%CI) were 2.66 (1.30-5.44) and 2.52 (1.01-6.29), respectively. Compared to HIV/AIDS ≤30 years old and receiving ART, those who were 31-50 years or >50 years old and not receiving ART were less likely to be satisfied, the OR (95%CI) were 0.36 (0.15-0.89), 0.32 (0.10-0.97) and 0.11 (0.01-0.90), respectively.@*Conclusion@#Satisfaction with follow-up management conducted by CHS in Hongkou district is relatively high. Age, residence, status of receiving ART and cumulative time of following up by CHS are significantly associated with satisfaction, suggesting that CHS should improve their professional abilities during follow-up management, as well as be more targeted and focus on different aspects.

Objective:To study the satisfaction of patients and medical personnel after the new health care re-form.Methods:Using questionnaire survey method, we interviewed patients and medical personnel in 4 comprehen-sive hospitals and 8 community hospitals.The number of questionnaire is 1001,the number of patient questionnaire is 499,the number of medical personnel questionnaire is 502.Results:The overall satisfaction of patients and medical personnel are good.The patient's overall satisfaction score was 3.95, the average satisfaction score of administrative supervision,hospital environment,service attitude and service ability is more than 4 points,the average satisfaction score of the knowledge publicity,drug supply,medical expenses,medical care system and medical processes is less than 4 points.The satisfaction degree of return of work of medical personnel is lowest,The satisfaction degree of work-ing environment is highest.Community hospital medical staff and patient satisfaction is generally better than general hospital.Conclusion:The overall satisfaction of medical personnel and patients is good in the Shanghai area.In the fu-ture,more should be done on knowledge publicity,strengthening diagnosis and treatment process optimizing,medical expense controlling,and the improvement of the social practice environment and working conditions.

OBJECTIVE:To provide reference for promoting the further implementation of national essential medicine system and rational use of drugs in community health service center. METHODS:Outpatients’information in Changqiao Street Communi-ty Health Service Center from Sept. 2,2009 to Nov. 25,2014 was collected to statistically analyze the drug variety,the number of prescriptions,the consumption sum and the DDDs before and after the implementation of essential medicine system and after the supplement of non essential medicines. RESULTS:Essential medicines occupied the main position of drug utilization in each stage, with substantial growth in respects of consumption sum and number of prescriptions;the sum of Chinese patent drugs and essential cardiovascular drugs occupied more than 50% of the total consumption sum and number of prescriptions,with higher growth rate;essential medicines occupied the top 10 in respects of consumption sum and DDDs,among which most were the drug varieties in“Shanghai supplement essential drugs in community health service center”(2010 Edition),with some overlap the varieties in“Na-tional Essential Medicine System”(2012 Edition);cardio-cerebrovascular drugs occupied the dominant position;the growth rates of cefotiam and atorvastatin took up the top 2 ranking in the list of consumption sum,while the growth rates of calcium carbonate D3 and irbesartan and hydrochlorothiazide took up the top 2 ranking in the list of DDDs. CONCLUSIONS:Essential medicine cata-log covers the common drugs in the community health service center,in which the rational use of Chinese patent drugs and antibac-terial deserves needs to be noticed. The clinical rational use of the essential medicines should be strengthened,the essential medi-cine catalog should be adjusted and non essential medicines should be supplied when necessary in order to truly meet the needs of patients and optimize the allocation of medical resources.

Objective To understand the current situation of dispensary setting and personnel of community health service centers in Pudong New District in Shanghai (hereinafter referred to as Pudong New District).Methods Questionnaire survey covering the 45 community health service centers in Pudong New District was used.Results 42 community health service centers were equipped with TCM dispensaries among the 45 total units (93.3%);12 of them adopted outsourcing mode (26.7%);the average area of community dispensary was (32.3 ± 17.3)m2;the average number of TCM practitioners was 3.5 ± 2.0 people.Conclusion Community health service centers need to increase the allocation area of TCM dispensary and improve qualification of TCM practitioners.

AIM: To observe the application of Suresight handheld auto- refractometer in measuring diopter of infants in Community Health Service Center. METHODS:Totally 836 cases ( 1 672 eyes ) from June 2013 to December 2013 were examined diopter of infants by Suresight handheld auto-refractometer in Community Health Service Center. RESULTS: Within 1 672 eyes of 836 infants were examined, 202 eyes were diagnosed ametropia, 38 eyes were suspicious, 240 eyes were transferred to the department of ophthalmology, the referral rate was 14.35%; 172 eyes were diagnosed ametropia, and the diagnosis rate of the referral patients was 71. 67%. Among 172 eyes, 46 eyes were provided with corrected glasses, accounting for 2. 75% of the number of screening, and 126 eyes were given intensive monitoring, accounting for 7.54% of the number of screening. CONCLUSION: Application of Suresight handheld auto-refractometer in refraction screening for infants in Community Health Service Center is convenient and effective. With two - way referral between community health service center and department of ophthalmology can monitor and intervene vision development of infants much earlier.

Objective To evaluate the impacts of the essential medicine system in Jiangsu Province on rational drug use in community health service centers.Methods 7667 outpatient prescriptions from 6 community health service centers in Nantong were selected by cluster sampling from January 2009 to December 2011,the part of SDUIs(Selected Drug Use Indicators)developed by WHO and the self-design indicators were employed to analyze rational use of drugs.Results After implementation of the essential medicine system,the average types of drugs per prescription decreased from 3.02 to 2.74,the rate of antibiotic drugs usage dropped from 61.43% to 51.2%,the rate of one antibiotic usage reduced from 33.94%to 28.58%,and two types from 20.43%to 15.8%,all with significant differences(P<0.001).The average expenses in single prescription arose from RMB 69.70 to 87.28,the rate of essential medicine usage increased from 81.22%to 85.60%,the injection utilization decreased from 31.14% to 14.13%,while TCM utilization ascended from 16.17%to 21.8%,all with significant differences(P<0.001).Condusion The essential medicine systems has positive impacts on rational drug use,but the expenses of per prescription are higher,and irrational antibiotic drugs are still a serious problem.

ABSTRACT

The objective of this study was to examine the inpatient bed(IB)allocation equity and utilization in Chinese city community health service centers(CHSCs).The data were derived from the Baseline Survey of National City Community Health Service System Building Project,which was conducted in 1917 CHSCs in 28 cities in 2007.The IB allocation was analyzed in terms of IB allocation quantity and distribution equity,and the IB utilization was analyzed by the IB utilization rate and average length of stay of the CHSC inpatients.The results showed that 49.3% of the CHSCs were equipped with IB; averagely,there were 45 IBs per CHSC,0.94 IBs per 1000 people,and 0.38 nurses and 0.57 doctors per IB; the IB Gini coefficient was 0.32; the IB utilization rate was 40.06%; and the average length of stay of inpatients was 12.24 days.The conclusions were that IB allocation among the population was equitable,but the number of nurse per IB was not up to the national standard; and the CHSC IB utilization was low as a whole,thus inpatient service was not the main health service for Chinese CHSCs.
